If you are filing an insurance claim and need to pay your deductible, please select from the following three options. Please note that upon selecting High-Cost ($5,500 - $1,000), Medium-Cost ($1,00 - $5,000), or Low-Cost ($100 - $1,000) you will have the option to select a more precise dollar amount. During the deductible payment process, a work authorization questionnaire will be presented for completion.
